Documents vital signs and symptoms as related by client or observed. Participants in client education and reinforcement. Enters information into Electronic Health Record.Essential Job Functions:Answers phones.Schedule appointmentsAssist in assuring access to community resources.Assist and provide reinforcement with patient teaching/ education.typing/printing correspondence for prescribers and/ or psychiatric services team members.Coordinates access to psychiatric services for associated or contracted agencies such as Blue Peaks, nursing homes, residential facilities, medical clinics, hospitals and community corrections.Provides required PCP Coordination of Care to allied medical facilities/ providers.Assists Psychiatric Staff with coordination of client communications regarding medications, including screening calls, sending correspondence, contacting pharmacies and other physicians, printing and distributing medication information.Assists psychiatric services team members by obtaining prior authorizations for patient's medications and communicating said information to pharmacies and patients.Works cooperatively with psychiatric services, medical records, clinical and support staff to assure department procedures are in line with agency policy, customer service, and good clinical practice.Responsible for various data entry, record-keeping and reporting required by agency administration.Meets performance standards as established by agency administration.Completing patient vitals, including height and weight for BMI, and document any complaints regarding information. All this information is provided to prescribers for review.Provides injections and administers oral medication following protocol.PM1Requirements:Education/Skill RequirementsEducation: Successful Completion of an Accredited MA ProgramExperience: Knowledge and/or experience working with individuals with mental illnessLicensure/Certificate/Registration: MA CertificateOther Skills, Knowledge and AbilitiesProficient computer skills including: Windows-based office programs, internet, email, EMR and ability to learn and use specialized agency computer programs.Successful completion of QMAP Certification (this is done after being hired).Strong customers service skills.Able to multi-task.Able to handle emergency situations calmly.Ability to work independently and as a member of a team to meet multiple demands in an organized, efficient and effective manner.Ability to work with the range of persons served by the department.Ability to trouble shoot, maintain a calm and professional demeanor under all circumstances.Flexible work hours as the job demands.Ability to maintain confidentiality.Be a facilitator of a healthy work place.Work Environment:General office environment; light physical work as needed, travel or errands outside the office.PM1PI197665051
