Position OverviewProvides care coordination services to clients, including but not limited to transportation, referral, vocational and psychosocial education, and linkage with community resources and supports. The Care Coordinator provides a high level of customer services to referring parties by:Promoting a timely access to appropriate careMaintaining ongoing tracking and appropriate documentation using the required technology and reports as may be required by SLVBHG administrationEnsuring patient/clients barriers to receiving services are addressedCommunicating status of referrals back to referring party in a timely mannerEssential Job FunctionsAssist patients/clients in problem solving potential issues related to the behavioral health system, medical health care system, financial or social barriers;Complete necessary health assessments to determine appropriate needs and linkage.Assemble information concerning patient/client's whole health needs and referral needs along with identify community resources for patient/client, family/caregiver(s) and community agencies.Interface and collaborate with other community agencies and resources to optimize service.Provide coordination and linkage to services appropriate to the age-specific needs of all patients/clients in accordance with the principles of growth and development for infant through geriatric life cycles.Build professional relationships and coordinate care with internal and external medical providers to ensure quality care and follow throughConsults and advises with other health care team members on methods of assisting patient/client and their families in overcoming social and emotional difficulties which may prevent effective health care, this may include psychoeducation, brief interventions or referral for further treatmentProvide behavioral health outreach to specific populationsBe the point of contact for partnering agencies in the community to collaborate within specific needs of entire populationCollect and interpret data on a regular basis to determine focus and direction of individual patient/client needsParticipate in multidisciplinary staffingConduct all professional activity in an ethical manner, maintain records accurately, adhere to timeliness in accordance with Center's standards, follow applicable policies and procedure, consult internally and externally with regard to patient careAdapt to change in the workplace and use change as an opportunity for innovation and creativitySecondary Job FunctionsOther duties as assignedPM2.

Requirements:
Education/Skill RequirementsBA in human service or related field3-5 years' experience in clinical or community resource settings; care coordination and/or case management experience is desirableProficiency in communication technologies (email, cell phone, etc.)Highly organized with ability to keep accurate notes and recordsLocal knowledge about and connections to community health care and social welfare resources is desirable.Other Skills/AbilitiesMust possess the ability to communicate with other in a clear, understandable and professional manner on the phone and in person; and demonstrated use of good written and verbal and nonverbal communication skillsMust possess the ability to interact with peers, executives, patient, families and other vendors in a manner that represents a respectful positive outcomesMust possess the ability to maintain regular, reliable and punctual attendanceMust possess the ability to learn an apply new information, knowledge and experience in a timely mannerMust possess the ability to be flexible and adaptable to changeMust possess the ability to work on multiple tasks and projects and to prioritizeMust possess the ability to work independently as well as a member of a team.Must possess effective organizational skills and attention to detail; effective follow-through, and commitment to excellenceMust possess proficiency with computers and Microsoft office programsMust Acknowledge client's rights on confidentiality issues, maintains client confidentiality at all times, and follows HIPAA guidelines and regulations.Work EnvironmentGeneral office environment; light physical work as needed: travel or errands outside the office.Salary - $21.00 per hourPM2PI160443077
