Job Description:

Stellar Living is seeking a dedicated and detail-oriented MDS Coordinator (Minimum Data Set) to join our compassionate team, supporting our mission to enhance the lives of seniors in our care facilities. As an MDS Coordinator, you will be pivotal in assessing and managing the clinical and financial needs of our residents through systematic and standardized assessments. Your precise documentation and care planning efforts ensure that our residents receive personalized and appropriate healthcare while maximizing the reimbursement from insurance providers. By contributing to our community, you help us provide the highest quality of independent living, assisted living, and memory care services. Conduct comprehensive assessments of new and existing residents to determine medical, psychological, and social needs. Prepare, implement, and review personalized care plans in conjunction with healthcare providers. Ensure compliance with federal and state healthcare regulations. Coordinate with healthcare team to assess resident responses to treatment and modify care plans as needed. Manage the MDS process including scheduling, coding, and completing assessments. Train and supervise staff on MDS procedures. Utilize MDS assessment data to optimize insurance reimbursements and compliance with reporting requirements. Proficient in MDS 3.0. Strong understanding of Medicare, Medicaid, and insurance reimbursement processes. Excellent organizational, communication, and interpersonal skills. Ability to use electronic health record systems efficiently. Critical thinking and problem-solving skills. Medical Insurance Dental Insurance Vision Insurance 401k Active RN or LPN/LVN license required. Certified in the Resident Assessment Coordinator - Certified (RAC-CT) program preferred.
