Job Description Job Description Summary: Anova Care, a provider of home care and home health services, is looking for a Speech Language Pathologists (SLP), to join our rapidly growing home health program. In this role, SLPs will render home health services to patients. Key Responsibilities: Conduct patient evaluations and develop individualized speech therapy treatment plans Provide skilled interventions to address speech, language, cognitive-communication, and swallowing disorders Monitor and document patient progress in the EMR and communicate updates to care teams Educate patients and families on therapy techniques, safety, and home exercises Ensure compliance with home health regulations, physician orders, and documentation standards Participate in care coordination, quality improvement initiatives, and interdisciplinary meetings Collaborate with clinical, administrative, and operations teams to support patient care Perform other clinical or administrative duties as assigned Position Qualifications: Master’s degree in Speech-Language Pathology required Current state licensure and ASHA certification (CCC-SLP) 1–3 years clinical experience in home health, rehabilitation, or outpatient settings preferred Strong assessment, treatment planning, and patient education skills Knowledge of home health regulations and documentation requirements Proficiency with EMR systems and Microsoft Office Suite Excellent written and verbal communication and interpersonal skills Availability during business hours with occasional after-hours/weekend coverage Hours: PRN, Pay Per Visit Work Type: In-person.
