*REHAB SOLUTIONS**Job Description: PRN Physical Therapy Assistant**Department: Physical Therapy**Supervisor: Regional Director of Clinical Operations**Job Description: Primary Location in Lamar, CO and surrounding areas.** Provide therapy services planned, delegated and supervised by the qualified Physical Therapist consistent with the patient's approved Plan of Treatment.* Observe patients during treatment, compiles data and report patient's progress to Physical Therapist.* Act as a liaison and maintains contact with patients, Physical Therapist, Clinical Manager(s), other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals.* Recommend to Physical Therapist possible modification to existing physical therapy treatment plans and training needs to achieve maximum results.* Maintain and submit documents as required by the Company and/or facility.* Accurately and thoroughly document patient care observations, interventions and evaluation on the day services are rendered. Assist qualified Therapist with preparation of patient progress notes.* Participate in Performance Improvement activities as assigned. Communicate information that enable collection and root cause analysis of data to identify opportunities for improvement.* Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when recipient's right to access is clearly established and sharing of such information is in the best interest of the patient.* Maintain/enhance clinical practice skills by attending, participating in/conducting staff development/continuing education program.*Qualifications *Degree from an accredited Physical Therapist Assistant Program (approved by the APTA)A minimum of six months physical therapist assistant experience preferredSNF experience a plusCurrent and unrestricted PTA licensureCurrent CPR certificationGood organizational and communication skills*Working Conditions** Interacting with the general public, often in adverse conditions; many patients and their families may be under emotional stress dealing with issues of illness, hospice, death and grief.* Frequently changing conditions in which one must be attentive and able to respond appropriately.* Constant necessity to respond maturely and effectively with patients, families, staff and other professionals.* Appropriately represent the facility in dress and demeanor.*Physical Demands *The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.* While performing the duties of this job, the employee is regularly required to talk and hear.* This position is very active and requires sitting, standing, walking, bending, twisting, kneeling, stooping, crouching, grasping and carrying.* The employee must frequently lift or move objects weighing over 35 pounds and occasionally objects up to 80 pounds.* Specific vision abilities required by this job include close vision and distance vision.Additional physical activities of this position include all of the following:Climbing Crawling Crouching Fingering GraspingKneeling Lifting Pulling Pushing ReachingStanding Stooping Repetitive MovementAn individual in this position will be required to carry or lift weights in this range: 25 to 50 poundsThe sensory and communication activities essential to perform this position include all of the following:Feeling Hearing Seeing Smelling Speaking TastingSpeaking English Understanding EnglishAn individual in this position may be exposed to all of the following:Inside environmental conditionsOutside environmental conditionsBloodborne pathogensPhysical HazardsChemical HazardsRespiratory Hazards''Work Location:* Multiple locationsWork Remotely* NoJob Type: Part-timePay: From $28.00 per hourMedical Specialty:* Geriatrics* Physical & Rehabilitation MedicineSchedule:* On call* Weekend availabilityCOVID-19 considerations:Yes, all state and federal guidelines.Education:* Associate (Preferred)Experience:* SNF: 1 year (Preferred)License/Certification:* CPR Certification (Preferred)* Physical Therapy Assistant License in Colorado/Compact (Preferred)Work Location: Multiple Locations
