Position OverviewUnder the direct supervision of Chief Clinical Officer, contributes to the assessment and management of the mental health wellbeing of the clients through collecting, recording and reporting objective/subjective data. To work hand in hand with the Psychiatric Nursing Staff in the coordination of care of clients/new psychiatric referrals. To be a facilitator of health through teaching and educating clients; directing and developing of the psychiatric nursing staff. Collaborates with psychiatric nurse practitioners and multidisciplinary professional staffs; providing psychiatric and psychological support for clients. Manages and coordinates care with outside agency partners to promote operative client care. Supervises the licensed practical nurses, medical assistants, and psychiatric peer specialists on staff and daily practices of the psychiatric department.Essential Job FunctionsSupervise licensed practical nurses, medical assistants, and psychiatric peer specialists on staff.Observe and evaluate staff performances; licensed practical nurses, medical assistants, and psychiatric peer specialists.Ensure treatment plans/CCAR's are completed, up to-date, and in compliance of agency requirements.Build providers initial schedule.Admin for the psychiatric nursing department; training of staff, manages referrals, manages intakes from outside agencies, PSO discharges and transfers, and coordinates care for hospital discharges/ES.Recognize, understand and respect the cultural, spiritual, religious backgrounds, beliefs, needs/rights to choice of the clients.Assist in assuring access to community resources.Assist and provide reinforcement with patient teaching/education.Consult and seek guidance from the Nurse Practitioner and/or Psychiatrist as necessary.Assist Psychiatric Nursing Staff with assigned ancillary duties.Reviews and prepares paperwork and/or electronic medical record prior to psychiatric visits, including up-to-date releases, consents, demographic information, diagnosis, treatment plans and other preliminary requirements.Requests records from or sends records to other providers as required.Coordinates access to psychiatric services for associated or contracted agencies such as Blue Peaks, nursing homes, residential facilities, medical clinics, hospitals and community corrections. Provides required PCP Coordination of Care to allied medical facilities/providers.Works cooperatively with psychiatric services, medical records, clinical and support staff to assure department procedures are in line with agency policy, customer service, and good clinical practice.Meets performance standards as established by Center administration.Secondary Job FunctionsCollect height, weight, blood pressure, pulse and waist measurements on interval visits of the clients when coverage is needed for nurses on staff.Collect data using psychological screening tools.Responsible for various data entry, record-keeping and reporting required by Center administration.Administer injections per the orders of the providers when coverage is needed for nurses on staff.Educate clients on Suboxone and review the consent and manage inductionsAssists Psychiatric Nursing Staff with coordination of client communications regarding medications, including screening calls, sending correspondence, contacting pharmacies and other physicians, printing and distributing medication information, and making computer or paper entries of prescriptions and refills.PM1.

Education/Skill RequirementsEducation: Successful Completion of an Accredited LPN ProgramExperience: Knowledge and/or experience working with individuals with mental illnessLicensure/Certificate/Registration: Current Colorado LPN LicenseOther Skills/AbilitiesProficient computer skills including: Windows-based office programs, internet, email, EMR and ability to learn and use specialized agency computer programs.Strong customer service skillsAble to multi-taskAble to handle emergency situations calmlyAbility to work independently and as a member of a team to meet multiple demands in an organized, efficient and effective mannerAbility to work with the range of persons served by the departmentAbility to trouble shoot, maintain a calm and professional demeanor under all circumstances.Flexible work hours as the job demandsPersonal vehicle and willingness to travelAbility to perform light physical work, such as stocking medications shelves, lifting 10 to 20 pounds.Be a facilitator of a healthy work place.Demonstrates leadership and supervisory ability and the willingness to work harmoniously with all personnel.Material and Equipment Directly Used: (including, but not limited to)Agency computer and records systems, copier, fax, telephone, email and voice mailWork Environment: General office environment; light physical work as needed: travel or errands outside the office.Salary $63,638 with no shift work requiredPM1PI160442222
