Qualifications: Master's degree in Social Work or related field licensable through the Kansas Behavioral Sciences Regulatory Board preferred. We provide service to these areas through our affiliates: TFI Family Connections in Oklahoma and Texas Family Initiative in Texas. Pathway Family Services is an affiliate in Kansas offering additional services to youth and families. Mainstream Nonprofit Solutions is an affiliate supporting the TFI Family of Companies and other Nonprofits. As a Case Manager for Family Preservation, you will promote the well-being of children/families in the Family Preservation Program, participates in staff meetings and trainings and coordinates with staff from other agencies involved in providing family preservation services. Manages a caseload of family preservation cases.
Coordinates the actions of the child and family team in providing services to the child/family.
Determines the services to be provided to the child/family and develops the case plan.
Accomplishes initial family assessment; individual, group and family counseling; and other professional services when capable. When not capable, contracts with mental health therapists to provide the required services.
Ensures the accuracy, content and completeness of case files.
Assess the need for placement, safety, family strengths and needs, and placement resources for the child and foster parents/kinship caregivers.
Plan for and facilitate and assist parents in providing a safe, nurturing environment for children in their care.
Maintains a close working relationship with public and private agencies involved in providing Family Preservation services/support.
Ensures the accuracy, content and completeness of child/family case records.
Coordinates Family Preservation services and requirements with public and private agencies.
Conducts case planning conferences at required intervals.
Provides information to the responsible agency on all matters related to the child/family.
Facilitates family preparation through training, family to family mentoring, family assessment, consultations, home visits, etc.
Maintains the accuracy and content of case records.
Works with the After Care Specialist in ensuring follow-up support services are accomplished.
Compiles monthly statistical data and reports and forwards to appropriate agencies.
Completes documentation and forwards reports containing descriptive, analytical and evaluative content.
Coordinates with intradepartmental staff, and staff from external public and private agencies, on the delivery of services to children and families.
Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position.
Advocates, promotes and practices cultural sensitivity and responsiveness in all day-to-day interactions.
Develops, promotes and practices teamwork in all activities.
Provides follow-up support services for the child/family when case plan goals have been met. Provides crisis intervention, when required.
Provides 24 hour, on-call support to the children/families served. Provides crisis intervention, when required.
Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position
Advocates, promotes and practices cultural sensitivity and responsiveness in all day-to-day interactions.
Develops, promotes and practices teamwork in all activities.
Must have knowledge, skills, and experience to assess the needs, assess safety, assess family strengths and needs, plan for and facilitate family preservation.
Must demonstrate skills in separation and loss issues for the child in care, family systems, conflict resolution, and mediation, understanding loyalty issues a child and kinship caregiver may face, and reunifying a child with his/her family.
