Description:Position OverviewProvides case management services to clients, including but not limited to transportation, referral, vocational and psychosocial education, and linkage with community supports.Essential Job FunctionsProvide direct client services and case management to BHG clients.Interface with other community agencies and resources to optimize service to BHG clients.Identify community resources and facilitate client referrals.Work directly with BHG clients and their family, friends, and partners to provide psychosocial support.Effectively present San Luis Valley Behavioral Health Group mission to San Luis Valley communities.Serve as a liaison and resource to San Luis Valley community agencies and area educators concerning persons with mental illness.Effectively communicate with other BHG personnel regarding client needs.Is responsible for record-keeping and reports as may be required by Center administration, and to subscribe to the code of ethics of this discipline.Utilizes Center technology for timely record keeping and report-writing.Meets performance standards as specified by supervisor and management staff.Provides assessment and treatment planning to persons with substance use issues.Facilitates dual diagnosis group counseling.Maintains records and reports as required by agency policy, OBH/ Signal, grant guidelines, and performance evaluations.Meets performance standards as established by Center administration.Keeps abreast of the rapidly expanding approaches and methods in substance abuse treatment.Secondary Job FunctionsWork closely with BHG management and personnel to create new client service programs and evaluate those existing programs.Assist in providing or arranging for individual, family, couple or group counseling.Participates in quality review and chart audits as requested.Assists in developing curriculum/materials and strategies in order to accomplish treatment objectives of the program.Complete trainings required to facilitate groupsMaintain CAC III certification required by DORA. Requirements:Education/Skill Requirements:Education & Experience: Minimum of Bachelor's degree. If Bachelor's degree, minimum of 3 years' experience in the human service field is required. If Master's degree, experience in the human service field preferred.Licensure/Certificate/Registration:Obtain QMAP CertificationValid Driver's LicenseCACIIIOther Skills/Abilities:Knowledge of and ability to provide case management service.Knowledge of and ability to apply crisis intervention skills.Ability to observe, report and keep record of client symptoms and progress.Ability to express oneself clearly and concisely, both orally and in writing.Ability to use supervision to enhance own performance.Ability to work independently and as a member of a team.Ability to work closely and cooperatively with external agency representatives.Creative problem-solving.Ability to perform light physical work, such as lifting 10-20 pounds, and the essential functions of the jobComputer skills, including Windows-based Office programs, internet, email, and ability to learn and use specialized agency computer programs-electronic health record.Knowledge of the dynamics of addiction substance use, its symptoms, treatments, and social implications.Work Environment:General office environment; light physical work as needed: travel or errands outside the office.Pay Rate $21.00/hrPM1PI159855240
